Related Service Logs Notice
Children who are eligible to receive special education and related service may have specific related services included in their Individualized Education Plan (IEP). Related services are added to an IEP if the IEP team determines that the specific related service is necessary in order for the child to benefit from his or her special education program. Illinois Public Act 101-0515 requires local education agencies to make related service logs that record the type and minutes of related services administered under a child’s IEP. Copies of related service logs will be provided to parents or guardians at the annual review of their child’s IEP or upon the request of the child’s parent or guardian.
